About the team

You will be a member of a dynamic team developing the model of care for Monash Health Renal Social Work working closely with the internal and external stakeholders to provide care founded on current evidence-based practice for this vulnerable population.

About the role

Monash Health Social Work Department is looking for an experienced Grade 1 Social Worker to join the team working with our renal dialysis unit at Monash Health. The position is a full-time position primarily based at Thomas St Community Health working with haemodialysis, peritoneal and home haemodialysis patient groups.

The work provides an opportunity to showcase your skills in working with people from diverse backgrounds with chronic illness. Experience in assessment, care planning, decision-making, and improving Quality of Life and Advance Care Planning for people living with renal disease together with an opportunity to provide education for future patients and their families requiring renal dialysis.

The position requires some travel between Monash Health sites and some on call requirements.

You must be eligible for AASW membership including continuing professional education and hold a current driver’s license and Working with Children Check.
