Our Integrated Community Health Team (ICHT) is part of our Community Services Directorate and conducts a wide variety of health related activities, from screening through to managing chronic conditions.  We are now looking for an experienced Podiatrist to join our Living Well team covering parental leave until 31 January 2019, ideally working four days a week between Monday and Friday. 

About the Role 

Our ICHT team uses a multidisciplinary approach in its work with clients who have a range of chronic diseases, including diabetes, cardiovascular and respiratory disease.  Based in the Greater Dandenong and Cranbourne area, our clients may not be readily engaged with health services and may also have insecure housing arrangements.

Working within a multidisciplinary model of care, you will provide individual podiatry treatment including assessments, reviews and care plans.  Providing patient centred care, this will include general podiatry biomechanics and delivering some group-based education, aimed at enhancing independence and improving health outcomes for our clients.

You have:

  • Current AHPRA registration as a Podiatrist and experience using self-management approaches to client care
  • Demonstrated ability to work both independently and effectively within a team
  • A keen eye for identifying new and better ways of doing things
  • Proven ability to meet operational and project targets 
  • Well-developed interpersonal, organisational and delegation skills

From a clinical perspective, you can demonstrate experience undertaking assessments, developing and coordinating care plans and delivering group-based self management education programs.  You have worked with socially and economically disadvantaged communities and you understand and practice the social model of health. 

 A current Driver's Licence and regular access to a reliable vehicle are also required.
